Medical Record Request

To request records from your work with me, you can send me a written request. Requests may be made via secure client portal, email, or in writing and include the details of your request and to arrange for me to send you an electronic release form. I will need to know the specific records you are requesting including dates as well as where and how you would like them to be sent. There is no fee for these requests.
